COVID-19 Protocol:

• If you are experiencing any respiratory symptoms (fever, shortness of breath, dry cough, runny nose, sore throat or loss of taste or smell), please do not schedule at this time.
• If you have been exposed to someone who has any of these symptoms or who tests positive for Covid-19 within the past 10 days, please call to reschedule your appointment.
• As of April 3rd 2023, mask are optional.

I would be happy to bill your insurance for this visit but you are responsible for knowing your acupuncture benefits prior to your appointment.  If you need help determining your benefits, please contact me prior to your appointment.  You can email your insurance information to t[email protected].

If this is your first appointment, please allow 75-minutes for your appointment.  Follow-up appointments are 60-minutes.

If this is your first visit with Teresa Jansen - Licensed Acupuncturist, you will need to complete paperwork prior to your appointment.  You are able to print paperwork from www.infinityspawellness.com.  If you are unable to print paperwork, please arrive 10 minutes prior to your appointment time and copies will be provided for your completion.   24-hour notice required for cancellations or a $50 fee will be applied to your account.  Please arrive at your scheduled appointment time.  If you are more than 15-minutes late, you may be asked to reschedule your appointment time and will incur a $50 fee for the missed appointment.
